In 2019, a high school alumnus’ schoolmate contacted me and said that he had a kidney stone attack. He complained of this problem for years and only recently had pain. Asked me whether it is better to see a Chinese medical doctor or a Western medical doctor.

Ten years ago, he took Chinese medicine prescribed by an old Chinese medical doctor at the Chi Fu Hospital for a few months with little change. On November 7, 2021, when I was washing up in the morning, I found a protruding piece on the left side of my neck, and I felt a foreign body when I pressed it with my hand, which was round, smooth and hard. I personally suspected a thyroid nodule mass, because I had been thinking too much and staying up late with anger continuously some time ago.

 

The symptoms are as follows.
Foreign body feeling in the throat, can’t swallow, can’t spit out, this symptom has been intermittent for three years, although not serious, but stretched. Belching often, fear of cold in the stomach and spleen, cold hands and feet, moodiness, and a very low menstrual flow in November.

So I prescribed myself Six Jun Zi Tang plus Ban Xia Hou Po Tang with Cat’s Claw Herb and Xia Ku Cao. The doctor said it was suspected to be a thyroid nodule and asked me to go to the hospital to have an ultrasound done to confirm it. I didn’t have an ultrasound done because I was already sure it was this disease and didn’t think it was necessary to have an ultrasound done because ultrasound is only used for diagnosis.

I just picked up the herbs at Tong Ren Tang, and after a few days of drinking this formula, the symptoms of cold and fire appeared, so I temporarily stopped taking the medicine. After a few days, I took it again, and this time I drank it for 3-4 days, but I had a cold and fire again, so I had to stop taking it again, thinking it was not right.

I found that the lump on the left side of my neck was getting bigger and harder, and it looked very obvious by naked eyes, because I only learned 20 lessons from the Yellow Emperor’s Classic of Internal Medicine in my course, so I was not knowledgeable about the prescriptions, medicinal properties and dosage, and I only knew that acupuncture could also treat it, but some points were inconvenient for me to put needles on myself, and some points were too scared to acupuncture so I gave up the idea of acupuncture.

On December 8, I sought the help of Master Guo and described my previous prescriptions and symptoms one by one, the specific symptoms are as follows.
There is a lump on the left side of the neck, sized like a walnut, sore and swollen when pressed, moving up and down when swallowing, touching it hard, feeling something blocked in the throat.

Bad sleep, dreaming a lot, dreaming of being chased, running, and climbing uphill… waking up very tired.

Right abdomen is feeling something hard, I personally feel it like a cluster of gas.

Eyes are easily sore and blurred.

Master Guo diagnosed the symptoms and gave me a prescription for 7 doses. I went to get the medicine that day and boiled it in a casserole. I had one bowel movement after 11:30 pm, a watery stool, and my stomach was not uncomfortable. After that, I went to bed and slept well that night, my night sweats were much reduced, I don’t remember if I dreamed or not, and my dry mouth and bitterness were much reduced the next morning.

 

After 7 doses of medicine, the lump on the left side of the neck was much smaller, about two-thirds of it had disappeared, sleep was normal at night, fear of wind was reduced, night sweats did not occur, the right abdomen became softer (I felt that my stomach had never been so soft), but there was still phlegm stuck, sometimes it could be vomited out, sometimes it could not, loose stools, poor appetite, easily sleepy, poor mental state.

After 5 doses of medicine, the appetite returned to normal, the lump could not be seen from the outside and became very small, about the size of peanut rice when touched by hand, the night sweats were completely gone, the sleep was normal, the mental state returned to normal, the blurred eyes and the acidity disappeared, but the tiredness and pain in the calf belly were still present when waking up.

The prescription was adjusted by Master Guo, and 7 doses were used.

This time, after a delay of three or four days for some reasons, I stopped taking the medicine for more than 10 days due to the onset of menstruation, and finished the 7 doses of medicine on January 14, and the lump on the left side of the neck was only a little bit left, which could not be touched without careful touching, but could be touched about the size of a green bean by careful touching, along with the white phlegm spitting out, and felt that the green bean-sized lump was slowly decreasing, and at present, except for the tiredness and swelling of the calf belly after waking up and having phlegm spitting out, there was no more discomfort.

 

Finally, I was given a prescription to solve the calf and knee problem by Master Guo, with 5 doses, and I will give feedback after I finish the medicine.

Though: I was ill this time, I was fortunate to have gained something from it. In the past, I would have been apprehensive and scared, but not this time, because I was sure that Chinese medicine could solve my problems.

The inspection diagnosis includes the complexion and morphology, but the focus is on the diagnosis of complexion and tongue. At the beginning of clinical practice, I often think that inquiring is the first of the four diagnoses, not able to understand the essence of diagnosis by inspection, but after a long period of practice, I can gradually understand the true meaning of “inspect and know is called God”, and the revelation of  “five complexions  diagnosis of ” is better than the appearance, the shape, and the motion.

 

1. Complexions diagnosis

Complexions Diagnostic is also called “five-complexion diagnosis”. The main focus is to observe the complexion of the face, and other parts of the body can also be analogous. Observation of facial complexion can measure the vitality of the viscera and blood, because the 12 meridians and blood are injected into the face, and there are corresponding changes under the disease.

 

Regardless of the five complexions, the common basic feature to distinguish the positive from the negative is that the positive is bright and terma, while the negative is obscure and exposed. At the same time, it also further relates to the eight levels of diagnosis to grasp the different nature of the lesions. For example, if the face is red, the redness on eyes and face indicates excesssive, and the redness on cheeks indicates deficiency; if the face is greenish, the pale face indicates cold, and the redness on face indicates heat. The Su Wen – Five Organs Generation Chapter determines the good and bad fortune of the patient according to the complexion of the face, pointing out that “those who are as green as the emerald feather can be saved, those who are as red as the crown of a chicken can be saved, those who are as yellow as the belly of a crab can be saved, those who are as white as the paste of a wild boar can be saved, and those who are as black as the feathers of a bird can be saved”; “those who are as green as the straw can not be saved, those who are as yellow as the hedgehog can not be saved, those who are as black as the coal can not be saved, those who are as red as the blood of a squid can not be saved, and those who are as white as the dry bones can not be saved.

 

The specific methods of the five organs in the corresponding parts in the face from the Nei Jing can be found in Su Wen – Theory of Stabbing Heat, which uses the forehead for the heart, the nose for the spleen, the left cheek for the liver, the right cheek for the lung, and the chin for the kidney. It is simpler than the “Ling Shu – Five Complexions” method.

 

Inspection of the eyes is an important aspect when inspecting the face. It is important to see whether the eyes are bright or dark, and to pay attention to their pathological characteristics. The floating eye cell is caused by wind or spleen deficiency and dampness, the swollen eye is caused by liver fire and phlegm stasis, the oblique view is caused by wind and fire, the straight view and upward view is caused by phlegm and stasis clouding the mind, the unequal pupil size on both sides is caused by water stasis in the skull and brain, the greenish eye and flexible vision is caused by liver exuberance, the red blood filled eye is caused by fire in the heart and liver, the deep yellow eye is caused by damp and heat in the liver and gallbladder, the pale white eye and face is caused by blood deficiency, the dark eye orbit is caused by phlegm stasis and kidney deficiency. If there are yellow spots in the eyes, it is due to phlegm and stagnation.

 

2. Tongue Diagnosis

Tongue diagnosis is mainly to observe the texture, coating, shape and dryness of the tongue.

 

As the veins of the five organs are all connected to the tongue, depending on the transmission of Qi, blood and fluids, so through tongue diagnosis we can observe the deficiency and excess of the internal organs to which the lesion belongs, the surplus and deficit of Qi and blood, the nature and depth of the disease.

 

We should pay attention to the color of the coating and the changes in the texture of the tongue. A light red tongue is normal; a light white tongue indicates deficiency of qi and blood; a red-red tongue indicates heat and yin; a blue-violet tongue indicates blood stasis.

 

Color of coating of tongue can be white, yellow or grayish black. White coating of tongue is mainly for exterior and cold, thin white and moist for normal people, thin white and dry for fluid deficiency exterior, thin white and slippery for cold and damp, thick white for dampness, phlegm and food accumulation. and rotten coating of tongue for dampness and heat and filthy turbidity up steaming.

 

Yellow coating is mainly for interior and heat, thin yellow for the evil heat is not yet strong, starting to spread into the interior; yellow greasy for damp heat or food accumulation of heat; yellow and sticky for dampness, phlegm and heat gum; yellow and dry, even burnt yellow for dry heat damage to fluids; light yellow and lubricant, mostly for dampness and phlegm aggregation.

 

If the coating is gray and black, a dry coating is mainly heat and a moist coating is mainly cold; a white, greasy, gray and black coating with a moist tongue is Yang deficiency and cold dampness and phlegm; a yellow, greasy, gray and black coating is dampness and heat for a long time,while a burnt, black and dry coating is heat and drained fluid.

 

At the same time, the dryness and moistness of the coating should be observed in order to know the excess and deficit of fluid in the body and its distribution.

 

① According to the external influenza and internal injuries, respectively, grasp the focus.
External acute fever: emphasis on the observation of the coating of the tongue, in order to understand the nature of the disease, hot and cold, and the progress and decline of the evil.
Internal injury chronic miscellaneous diseases: emphasis on the observation of tongue texture, in order to understand the deficiency or excess of qi and blood in the internal organs, the characteristics of the symptoms and the attributes of the disease mechanism

 

②The same coating of the tongue, in terms of the deficiency and excess, heat and cold, there is a difference between the lightness and the depth, for example: white greasy tongue is cold and damp, but the degree of white greasy varies greatly, we must judge the lightness of the dampness; the same yellow greasy tongue, the disease is damp-heat, but deep yellow thick greasy for deep damp-heat interior, the central accumulation of rotting is damp-heat and stagnant, if dry and less fluid, there is an opportunity to hurt Yin.

 

③ distinguish the true and false attributes of the disease mechanism: the same black coating, if black and lubricating, is the strong Yin cold interior, with the trend of exceeding water transforming to fire; if black and dry and cracked with thorns, it is the evidence of extreme heat.

 

④ Pay attention to the mixed pathogenesis: If the tongue is yellow and white, it is mostly mixed with cold and heat, or the warm evil is transmitted from the exterior to the interior. If the texture of the tongue is light and less fluid, it is mostly a sign of deficiency of Qi and Yin.

 

⑤ identification of the correspondence of five organs in the tongue: “Bi Hua Yi Jing” said that “the tip of the tongue is for heart, the middle of the tongue is for spleen and stomach, the edge of the tongue is for liver and gallbladder, and the root of the tongue is for kidney”. This can be for your reference in diagnosis.

 

The tongue is closely related to the heart and stomach pathology, because the tongue is the seedling of the heart, the color and texture of the tip of the tongue can mostly show the pathology of the heart meridian; the tongue is the gastroscope, the tongue coating is produced by the steam of the spleen and stomach, the color and texture of the coating can mostly show the pathology of the spleen and stomach.

 

Observation of tongue shape is also an important part. The old and young are observed to identify evil or deficiency; the fat and thin are seen to identify yang deficiency, dampness or deficiency of yin and blood; the tongue with cracks is for the loss of essence and blood and yin and fluid; the “Strawberry like” tongue is for the heat of the internal organs, and the tooth marks on the tongue are for the deficiency of yang and dampness interior. Atrophy of the tongue is extreme heat and yin damaged, and prolonged atrophy is deficiency of qi and blood; Stiff tongue is heat and yin damage, or wind and phlegm blocking the veins; Crooked tongue is internal wind and phlegm, and stasis blocking the veins; Curled tongue is cold and dense, or phlegm blocking the root of the tongue, or heat damages fluids; Coarse swelling and bruising of the veins under the tongue is blood stasis.

Ten days ago, I suddenly received a request for help from a friend in Shandong province on Wechat, complaining of stomach pain and menstruation at the same time, which was black and dirty and still dirty at the end. She had not eaten for several days, only congee, no appetite, feeling bitter in the mouth, stomach pain involving the pain round the waist. Its like a wave which is quiet normally, when seizure will be in pain from light to heavy then to light.

 

She asked if she could take any Chinese medicine. I didn’t know her situation well enough to give any advice, so I refused at that time.

 

She then went to the hospital to see her doctor, who asked her to be hospitalized for a detailed examination before making further treatment plans. After examination, it was found that there was a 5.7*4.6 mass in her uterus and the ultrasound showed a smooth edge of that mass.

 

The doctor recommended an immediate surgery, but as the fever had been persistent, the inflammation needed to clear before surgery could be performed. The patient was allergic to the antibiotic cephalosporin and was given levofloxacin infusion for three consecutive days, which did not reduce the fever. The fever was usually in the afternoon, 37-38°C.

Listening to the voice: If the patient’s voice is hoarse, or he even loses his voice, the sudden onset of violent illness is more likely to be excess syndrome, while the accumulation of long-standing illness is more likely to be deficient, or health qi is deficient while pathogenic qi is excessive . Delirium and wild speech are excessive, and are mostly caused by phlegm and fire-heat stasis; Zheng sound and solitary speech are mostly caused by great injury to heart qi, or qi depression and phlegm obstruction; strong tongue is mostly caused by wind and phlegm obstruction.

Listen to the vomiting: should be distinguished from vomiting when eating, vomiting at night while eating in the morning, vomiting slowly or acutely so as to distinguish the deficiency and excess; Hiccup loudly and frequently is excessive, lowly,weakly and intermittently deficient.

 

Smell the odor: breath comes with sour and rotten odor means gastrointestinal stagnation; putrid smell in the mouth, gum erosion means ulcerative gingivitis; coughing and vomiting fishy pus, blood and phlegm means pulmonary carbuncle; warm-heat disease has bad breath or sweat odor for epidemic poisoning; the body has corpse odor means the danger of organism failure, etc. In combination with the identification of disease, we need to pay attention to urine odor in the mouth of uremia, liver coma liver odor, diabetic ketosis rotten apple smell, coughing and vomiting blood patients with the smell of blood.

When Master returned from Myanmar, he had a high fever, severe fever and chills, a heartbeat of over 130, painful untouchable skin, painful swollen joints in the limbs, swollen face, eye droppings, eyes that could not be opened, and small pustules on the skin all over his body.

 

He invited an authority from the Western medicine immunology department to look at the situation and said that the master’s situation was very serious and could lead to sepsis and ulceration of the whole body.

 

I immediately called my teacher, described the master’s symptoms, teacher asked, “Feel to see if the old monk sweat out of the body? Is there nausea and vomiting?”

 

I said: there is a little sweating, no vomiting.

 

Teacher said, “Do not care what Western doctors say the name of the disease, how serious, do not be nervous, its just a small problem to TCM classical formular. A dose of formula with six bowls of water boiled into two bowls, take the first bowl before meal, within three hours if the symptoms go, then stop taking, if the symptoms are still there, then take the second bowl. ”

 

According to my teacher’s instruction, I gave the master a prescription to get the materia medica and decoct, and fed him the first bowl, the symptoms of cold and heat went away 80%, and then took the second bowl, and the cold and heat were eliminated.

 

……… However, because I was too nervous, I made a mistake in the first dose on ephedra as 25 grams, the same as the Cinnamon Twig.  As a result, the master’s condition improved very significantly, but his lips got dry and ruptured!

 

I have not yet mastered the classical formula well, so I had to ask my teacher for help again. Teacher asked about the prescription and pointed out that it was because I had reversed the dosage of Ephedra and Cinnamomum that the patient had dry lips, otherwise the patient would have been able to finish the work in one dose.

 

Regarding the master’s severe joint pain, the teacher suggested to add 15 grams of Rhizoma Atractylodis Macrocephalae, 15 grams of Radix Pseudostellariae, and 25 grams of Divaricate Saposhnikovia Root to the original prescription. Before I left, I went to the mountain to visit Master again, and his skin was reborn like a skin change. Master told me that after the first dose, the skin of his toes could be taken out intact like a thumbtack.

1. Ask about sleep
How is your sleep?
Do you sleep until dawn?
Do you wake up regularly every day?

Normal: sleep until dawn

2. Appetite

3. Stool
Are you constipated? Do you have stool every day? What is the color of your stool? Is it loose? Smelly or odorless?

Thin: watery in the middle energizer and intestinal tract
Hard: Yang-ming internal organs are solid
Downward flow of clear grain: spleen Yang deficiency

 

4. Urine
What color is your urine? Do you urinate frequently? Or can’t urinate? Or no urge to urinate? How many times a day on average?
Normal is: light yellow urine
Yellow or dark yellow urine: there is still dry stool in the intestines
Clear, white urine: cold inside, not enough heat in the small intestine

 

5. Ask about thirst
Are you very thirsty? If thirsty, what temperature of water do you want to drink most? If you are not thirsty, do you often forget to drink water? Or no matter how much you drink, you can not quench your thirst?
Normal: Sometimes feel thirsty
Not thirsty: water stops in the upper energizer or there is still unclean food left in the stomach

Thirsty and want to drink ice water: internal heat

6. Ask about sweating

Normal: sweat easily in the heat or exercise
Excessive sweating: deficiency of surface Yang, unable to protect the surface
No sweating: the heart cannot control five fluids because of the impaired heart function

 

7. Ask about the body is feeling hot or cold when sleeping
normal is: no special feeling.
Cold all over the body: Yang deficiency
hot on top and cold underneath: yin and yang divided
Cold and heat together: Yin and Yang fighting each other
Hot and dry all over the body: Yang does not enter Yin (chronic low fever)

 

8. Temperature of the feet
Do you usually feel very hot or very cold? Are your hands and feet cold?
Normal is: warm
Ice: insufficient heart yang, the heat from heart to the small intestine is insufficient

9、Menstruation -must ask about menstruation for womenIs your menstruation delayed or early every time? Is it painful? Have you ever had a baby?

Poor physical strength: Insufficient heavenly yang so that lung yang cannot reach down to the kidneys. Or Insufficient innate kidney yang. Therefore, feeling fatigue all the time.

 

He who can know Yin and Yang can be a doctor. In the diagnosis and inquiry, the great method of yin and yang is applied at all times to determine the disease.

 

In fact, the ten inquiries in Chinese medicine are to collect the deficiency or excess of yin or yang of the patient, and only when the deficiency or excess of yin or yang is known will there be a direction to prescribe medicine.

 

What can be seen is yin, and what cannot be seen and functional is yang. When yin and yang are in harmony, yang is submerged in yin, so the yang side is cool, such as the back of the hand and forehead, while the yin side is warm, such as the palm of the hand is warm. When yin and yang are out of balance, if yang does not enter yin, the body will feel hot and dry; when yin and yang are in harmony, the body will feel hot on top and cold on the bottom; and when yin and yang are fighting each other, the patient often has the phenomenon of both hot and cold, so from the change of yin and yang, we can also know whether the patient is in the aggravation or decrease stage of the disease.

(Qi) Cun of right hand : upper energizer ~ lung (large intestine)
Guan: middle energizer~ spleen (stomach)
Chi: lower energizer ~ kidney (triple energizer, endocrine)
(Blood) Cun of left hand: upper energizer ~ heart (small intestine)
Guan: Middle energizer ~ liver (bile)
Chi: lower energizer ~ kidney (bladder, urinary system)

4, Float,middle and deep. The three degrees of pulse taking force method
Float to take: fingertips, gently pressed against the skin of the pulse above
Middle take: fingertips, press some into the flesh
Deep to take: fingertip, press to the bone to the whole pulse, as if it was pressed off, stick to the bone.

5、The pulse of the four seasons (the unity of heaven and earth)

 

Take the analogy of the natural world: blood vessels are just like rivers,whose flow is like the change in the 4 seasons in the year:

 

In the spring, the snow just meltes, the river begins to trickle inside, the water little by little begins to come out, and then more and more water; in the summer time, the water will be more vigorous; in the autumn time, the level of river begins to lower. In the winter time, the surface of river sometimes freezes, the water is at the bottom.

Spring pulse ~ string pulse, like a string or a water pipe has not been full, so it is relatively thin, but because the water pressure is increasing, so there will be a little tightening,with the feeling of strangulation, just like a string.

Summer pulse ~ flood pulse, the blood flow is relatively large, like the river water flow is more vigorous when there will be a larger pulse. In summer, the pulse will be beating slightly faster, bigger flow feeling.

Autumn pulse ~ floating pulse, blood vessels in the summer has been propped up, but the water begins to lower, so will feel floating outside,while empty inside. Ancient books called this feather pulse, like touching a feather, that is, will get the blood vessels very wide, but press down inside empty, as if this pulse is something floating on top of water

The pulse in winter ~ deep pulse, the cold weather in winter, people’s nutrition is storaged inside, the pulse is sinking down.
