Case of uterine mass, menstrual stagnation and abdominal pain

Ten days ago, I suddenly received a request for help from a friend in Shandong province on Wechat, complaining of stomach pain and menstruation at the same time, which was black and dirty and still dirty at the end. She had not eaten for several days, only congee, no appetite, feeling bitter in the mouth, stomach pain involving the pain round the waist. Its like a wave which is quiet normally, when seizure will be in pain from light to heavy then to light.

She asked if she could take any Chinese medicine. I didn’t know her situation well enough to give any advice, so I refused at that time.

She then went to the hospital to see her doctor, who asked her to be hospitalized for a detailed examination before making further treatment plans. After examination, it was found that there was a 5.7*4.6 mass in her uterus and the ultrasound showed a smooth edge of that mass.

The doctor recommended an immediate surgery, but as the fever had been persistent, the inflammation needed to clear before surgery could be performed. The patient was allergic to the antibiotic cephalosporin and was given levofloxacin infusion for three consecutive days, which did not reduce the fever. The fever was usually in the afternoon, 37-38°C.

She expressed that she sweats easily on her chest, sweats on her upper body when she has fever. She has no appetite, her stomach hurts, her belly is so tight that it hurts when she tugs on her clothes, she is more comfortable when she lies down, but it hurts badly when she stands, and her right breast hurts a little occasionally.

After a week of examination and infusion in the hospital, the patient was still not well and was so upset that she didn’t want to have surgery, so the doctor could only discharge the patient to go home for three days and then follow up.

In WeChat messaging with me are crying, feel so helpless, too much pain feel very uncomfortable, she are ready to die, and said to her husband if she died, he can marry again, as long as her children are raised well. She told me to help her no matter what.

I asked her to send me the Ten Inquiries of Chinese Medicine, a photo of her tongue, and a small video briefly describing her condition.

I wrote a prescription and told her to soak the medicine in a small cup of wine and water for 20 minutes and then boil it for 40 minutes, with one of the medicines, mannitol, to be added when drinking every bowl of the medicine.

She may have diarrhea after taking the medicine, but don’t worry. If three doses are not effective, seek another highly skilled doctor.

She gave feedback after taking the medicine: when she drank the first bowl, the medicine was very hard to drink and she wanted to vomit, and by the afternoon she still had a fever, and the strangury hurt two or three times around seven o’clock, but the pain was much less. I instructed her to continue to drink the next two doses.

This morning, she asked me if she should continue to drink the medicine since she had finished the three doses, her body temperature had returned to between 36-37℃, her stomach didn’t hurt anymore, she didn’t have to cover her stomach when she went to the bathroom, she could walk more easily, and her appetite had improved. I asked briefly and wrote another prescription to close, and told her to go for an ultrasound after drinking the five doses.
